FREQUENTLY ASKED QUESTIONS

  • No, IESA/IHSA sports physicals MAY NOT be used as the required school health exam, per IDPH. However, school physicals may be used as sports physicals.

  • Yes.

  • Children need physicals at ages 5, 11, and 15.

  • No.

  • You will need to confirm with your organization if they require a specific form. We provide BSA & GSA forms below.

  • No, we cannot file with your insurance; however, we will provide you a receipt to send to your insurance/HSA/Flex with all the necessary information and codes.

  • Yes, payment is expected at time of service. We take cash, credit, debit, HSA card, and Flex card.
